ForaCare D40d Companion App for iOS and Android
The FORA® D40D is a dual-function blood pressure and blood glucose monitoring system, designed for comprehensive hypertension and diabetes care. The device can read out results in English and Spanish, uses smart averaging technology (AVG mode) and irregular rapid beat (IRB) technology. Equipped with Bluetooth connectivity, the meter integrates with the all-in-one MedM Health Diary app for iOS and Android, allowing for seamless data transfer and real-time monitoring. Measurement data from both blood pressure and blood glucose tests can be automatically synced to a smartphone or tablet, with the option to back up the data to MedM Health Cloud for secure storage.
Through the MedM Health Diary app, users can track their blood pressure readings and blood glucose levels, and tag measurements with additional notes, such as feeling (good, normal, bad), body position, measured arm, and meal-related tags (before meal, after meal, fasting, snacks, bedtime). All data is encrypted and stored securely in MedM Health Cloud, ensuring privacy and compliance with data protection regulations. The app also allows users to set reminders for taking measurements and medications, further supporting consistent health management.
MedM Premium Membership unlocks additional features, including syncing with Apple Health and Google’s Health Connect, sharing access to health records with trusted users, and customizing thresholds for readings, with notifications when thresholds are exceeded.

How to Connect ForaCare D40d with MedM App
- Open the app menu -> select My Devices -> tap Add Device
- Press and hold the "<-" (left arrow) button on the Fora D40d and wait for the Bluetooth indicator to start flashing
- Once the app discovers the Fora D40d unit - select it, configure device settings, and tap Add to My Devices
- Upon successful pairing a corresponding popup will appear
- The paired Fora D40d meter will be listed on the My Devices screen
- Make a measurement on the Fora D40d to collect new data in the app
Please note that the app has to be running in foreground or background (configurable in settings) to receive new readings from the Fora D40d.
